This declaration covers all donations I have made since 6th April 2000 and all donations I make hereafter. I wish these donations to be treated as Gift Aid donations. I understand that if I no longer pay an amount of income tax or capital gains tax equal to the tax reclaimed on my donations (28p for every £1 given), then I must notify you.
